Concert Choir Registration

This item is for registration for Concert Choir for a single chorister for the 2008-2009 performance year.

All families must accept the ChildrenSong Member Agreement. By indicating "I accept the ChildrenSong Member Agreement" in the selection box below you are expressly indicating your understanding and acceptance of the ChildrenSong Member Agreement.

If you choose to pay in installments, tuition and fees are due according to the follwing schedule:

• August 18, 2008 - At least ½ of tuition fee and all of concert attire fees, as applicable
• September 8, 2008 - Remaining tuition payment (unless application pending for scholarship or payment plan)

If you are requesting a scholarship or extended payment plan, do not use this form.  Instead, please print out the Application for Scholarship and Extended Payment Plan, complete the form, and send it to ChildrenSong according to the instructions.
