2011-2012 Prelude Choir Registration

This item is for registration for Prelude Choir for a single chorister for the 2011-2012 performance year.

All families must accept the ChildrenSong Member Agreement. By indicating "I accept the ChildrenSong Member Agreement" in the selection box below you are expressly indicating your understanding and acceptance of the ChildrenSong Member Agreement.

If you are requesting a scholarship or extended payment plan, do not use this form.  Instead, please see the Parents section of our site for the Registration Packet which includes an application for scholarship and extended payment plan. Complete the form, and send it to ChildrenSong according to the instructions.
